Decentralization remains a major strategy in implementing Universal Health Care (UHC) in the Philippines; however, variations in local governance capacity may influence healthcare access and financial protection across communities. This study aimed to explain how decentralized local health governance shapes healthcare access, service utilization, health literacy, and financial risk protection during UHC implementation in a rural Philippine province.
